Form Completion
Forms for Licences, Insurance Claims/Applications, Employment Medicals, General Applications and the like, may incur an out-of-pocket and/or non rebated expense.
Please send a copy of the form you wish to be completed to reception@geelongmarketplacemc.com.au at least 24 hours prior to your appointment time. The GP will assess:
A) if the forms can be completed
B) appointment length (which may require your appointment to be rescheduled to accommodate)
C) approximate cost for the appointment
A Receptionist may be in contact with you regarding the appointment to discuss further, as needed.
Where no forms are received prior to your scheduled appointment, please be advised that the GP may not go ahead with your appointment.
Insurance Claims/TAC/Work Cover
Please ensure that you have a claim number for your incident. Until we receive a letter from your insurer accepting liability for your claim, payment will be the patient’s responsibility.

After Hours Needs
When our Practice is closed and you need medical assistance – please call 000 in an emergency or make your way to your nearest Priority Care Centre (North Geelong) or Emergency Department. For non-urgent care, you may like to contact Nurse on Call on 1300 60 60 24 or the National Home Doctors Service on 13 74 25 (13 SICK).
