Records Transfer from MedicAid Geelong

As of August 2024, Geelong Marketplace Medical Centre, took over the administration of patient records from MedicAid Geelong, which has now permanently closed.
Your information is secure and can only be accessed by select persons within our Centre, for the purpose of transfer of records to your new Medical Provider.
Our new Centre, at the Ryrie Health Hub, continues to take on new patients, and can easily have your medical history transferred to your new record with us, at no cost.

Due to the format of each file received, transfers to external Medical Centres/Providers will incur a cost of $25 (per request), for retrieval, administration and postage.
Please be advised that your record cannot be given directly to you, in person, under any circumstances, including a printed health summary.
The Transfer of Records is a formal process, requiring specific steps to be taken to ensure the correct use of your information, as per the Health Records Act 2001.

Transfer Process to Geelong Marketplace Medical Centre

Along with a New Patient Form, we will provide you with a Transfer of Medical Records form for each patient over the age of 16, to be transferred to our Centre.
This obtains your consent for the transfer to occur.
Patients under the age of 16, of whom you are a parent or legal guardian, can be included on your transfer form, or a separate form signed by you.
No verbal or written requests for transfers (via email or otherwise) will be actioned.
Once transferred, a copy of the ‘Transfer of Medical Records’ form will be kept on your file as evidence of your request and consent.

Transfer Process to external Medical Centres/Providers

Your new Medical Centre/Provider will need to provide you with a Transfer of Medical Records form for each patient over the age of 16, to be transferred to their location.
Patients under the age of 16, of whom you are a parent or legal guardian, can be included on your transfer form, or a separate form signed by you.
Once the form is completed, showing your signed consent, your new provider should fax or email to Geelong Marketplace Medical Centre, requesting the transfer.
Our Centre will then contact you via phone to arrange/obtain payment for your record transfer.
Once paid, you record request will be processed and sent via registered mail to the new location.
No verbal or written requests for transfers (via email or otherwise) will be actioned.
A copy of your request to transfer your file to another location will be kept at Geelong Marketplace Medical Centre, in line with the Health Records Act 2001.
